US Passports

For United States Persons

To Expedite a New Passport or stolen/lost  passport you need:
  • 2 Passport-sized photos.
  • Original/Certified Birth Certificate or Naturalization Certificate or a Passport issued over 15 years ago or Current Passport
  • Copy of valid Driver's License as Proof of Identity (Children – copy of mother’s and father’s proof of identity)
  • Completed Passport Application Form DS-11(Do not sign this application until instructed by clerk). 
  • For those who have lost a valid passport, you must also complete Lost Passport Form DS-64
  • Proof of Departure - Original flight itinerary from the airline, copy of airline ticket or a corporate letter stating U.S. departure date  (Departure must be within 14 days for those requesting a passport in less than three weeks).
  • Check/Money Order, payable to U.S. Passport Services.  Do NOT write checks until instructed to do so by clerk.
  • 2 Letters of Authorization (attached) – one goes inside the envelope, one outside.

Instructions:

Take the items for the Passport Office to an authorized post office or clerk of court, indicating that your passport application will be hand-delivered to the passport office.

Ask them to execute the application and seal them in an envelope marked "TO BE OPENED BY PASSPORT OFFICE PERSONNEL ONLY".  Do not leave sealed envelope with clerk. You must present your original proof of identity to clerk.  All applicants must appear in person at the post office or the courthouse, including children and infants. 

Minors must be accompanied by both parents and legal guardians with acceptable proof of ID.

Note: Sealed envelope expires 5 days after being sealed by clerk of court or postmaster.

Note: Do not go on the post office until you not check all documents with agent’s manager. 

Once you’ve complete all the above steps send the sealed envelope to Russian Visa & Passport Center. Please also send:

  • Payment (Russian Visa & Passport Center processing fee – please see bottom of this page)
  • Information sheet (attached)
  • Prepaid self-addressed envelope for return mail or $30 for FedEx

PROCESSING TIME: UP TO 2 WEEKs
PROCESSING FEE: $450 (DOES NOT INCLUDE DEPARTMENT OF STATE FEE)

 

PROCESSING TIME: UP TO 1 WEEK
PROCESSING FEE: $550 (DOES NOT INCLUDE DEPARTMENT OF STATE FEE)

 

PROCESSING TIME: UP TO 3 BUSINESS DAYS
PROCESSING FEE: $650 (DOES NOT INCLUDE DEPARTMENT OF STATE FEE)

To expedite a Passport Renewal or damaged passport, you need:
  • Two recent identical passport-sized photos (Photos must be taken in front of a plain light background, and must be no more than 6 months old)
  • Your most recently issued U.S. Passport
  • One completed application form DSP-82 (you may download this form at travel.state.gov), it is also available at authorized post offices (Original marriage certificate or court order if your name has changed)
  • If a passport was damaged, applicant must submit Statement of Mutilation form to explain the circumstances surrounding the damage. Answer the questions clearly and concisely as if speaking directly to a US State Dept. official. Questions regarding previous passport number, expiration date, etc may be left blank if the information is not available Click here to complete the Statement of Mutilation Form
  • Check or money order payable to Russian Visa & Passport Center.

PERMIT TO RE-ENTER THE U.S. (TRAVEL PASSPORT)

U.S. Immigration and Naturalization Service (INS) issues US Passports for non-citizens permanently living in U.S.

In order for Russian Visa & Passport Center to apply to INS for a Permit of return travel document for you, we would ask you to submit the following paperwork:

  • Copy of your Permanent Resident Card (both sides)
  • Copy of your driver's license or other photo ID
  • 2 photos (passport size, for travel document)
  • Application form I-131 filled out signed (please fill out part #1 and #8, not #9!) you may download this form at travel.state.gov
  • Previously issued travel document (if applicable)
  • payment in the amount of $830

PROCESSING TIME: 6-12 WEEKS

REFUGEE TRAVEL DOCUMENT

If you have a refugee status, you should request Refugee US Passports

In order for Russian Visa & Passport Center to apply to INS for a Refugee travel document for you, we would ask you to submit the following paperwork:

  • Copy of your a he white card both sides
  • Copy of your driver's license or other photo ID
  • 2 photos (passport size, for Refugee travel document)
  • Application form I-131 filled out signed (please fill out part #1 and #8, not #9!) you may download this form at travel.state.gov
  • Previously issued travel document (if applicable)
  • payment in the amount of $455
